When calling to schedule an appointment, please have your insurance information available. If you have to cancel an
appointment, we ask that you please call no later than 24 hours in advance of your appointment. We do understand
that emergencies occur and that it may not always be possible to give advance notice, however, you may be responsible for
payment for appointments not cancelled in advance.
